Required Documents for 2024-2025 Interns Starting Training on November 1, 2024

·  Four copies of the personal ID card

·  Two copies of the birth certificate

·  Nine passport-sized photos (6*4)

·  A copy of the graduation certificate, or a passing grade certificate

·  A declaration of marital status and a Form 6 from the Social Insurance and Pension Authority.

·  Military service status (for males)

·  Vaccination against the Hepatitis B virus

·  A plastic envelope to store the documents and six transparent sheet protectors.

• Postal Visa.

Second: Biometric Enrollment:

Visit the fingerprint office to have your facial biometrics recorded by October 31, 2024. Submit the aforementioned documents to the Medical Affairs Department by October 31, 2024. Starting from November 1, 2024, interns must use their fingerprint device to sign in and out.

Third: Logbook:

Reserve a Logbook from the Medical Affairs Department.
